Residents in Independent Living and/or Assisted Living Communities require skilled home health treatments to maintain their strength and independence. Med1Care partners with many local Independent Living and Assisted Living Communities to provide individualized and highly skilled home health services that include aide and companion services to keep members in your community independent. Med1Care partners with many local Skilled Nursing Facilities to help ensure continuity of patient care at home after discharge.
