Serving Findlay and Hancock County
When a parent in Findlay starts needing more help than the family can cover, the phone calls usually land on one person. Call us instead. Med1Care runs a Findlay office right on South Main Street, and our aides, nurses, and physical, occupational, and speech therapists make home visits across Hancock County on your doctor's schedule.
A Med1Care person answers 24/7/365, never an answering service. Office hours are 8:30 to 4:30, Monday through Friday. Referring a patient? Send a referral here.
What we do here
Your doctor sets the orders. We staff the plan from right here in Findlay.
Personal care and support
An aide from our Findlay office comes on a schedule that fits the household, handles the bathing and the meals and the light housekeeping, and gets backed up by a licensed nurse who supervises the plan. We match the person to the home, not just to the shift.
Skilled nursing at home
RNs and LPNs from Med1Care handle physician-ordered care in Hancock County living rooms. Wound care after a hospital stay, IV medications, the whole list below. Your doctor writes the orders, and our nurses carry them out and report back.
Physical, occupational, and speech therapy run through our in-home therapy team, and your doctor can add them to the same plan of care.
Paying for it
When you qualify, Original Medicare pays nothing out of your pocket for covered home health visits. Medical equipment is the exception, and that runs 20 percent of the approved amount after your Part B deductible. Medicare Advantage plans set their own cost sharing and often want prior authorization, so the answer there depends on your plan.
Qualifying takes a few things at once. A doctor has to certify the need after seeing the patient face to face, the care has to be part-time or intermittent skilled nursing or therapy, and the patient has to be homebound. We're a Medicare and Medicaid certified agency, and we also work with Ohio Medicaid, Passport, waiver programs, and most private insurers.
Don't fill anything out yet. Phone us at 419.422.0305 and we'll check your plan while you're on the line, then tell you what applies to your situation rather than the average one.
There's no Medicare payment for help with bathing, dressing, meals, or housekeeping when that's the only care somebody needs. It covers aide visits alongside ordered skilled nursing or therapy, not on their own. Private duty care is available if you'd rather arrange and pay for help directly. If daily help is what your family is after, say so on the first call and we'll tell you what your options actually are, including the ones that aren't us.
Nothing here gets sent anywhere. It's a gut check before you call.
Check the boxes that apply. Either way, the next step is the same phone call.
This isn't a coverage decision. Medicare and your insurer decide what's covered. Our intake team can walk you through it in about ten minutes.
The record
CMS scores every certified home health agency in the country on the same measures and publishes the results. Ours are below. Anyone can look them up.
Two more worth knowing. Med1Care beats the national rate on discharge to community and on potentially preventable hospitalization, both risk-adjusted by CMS. Every figure here is published on Medicare Care Compare under CMS certification number 368362, where it refreshes quarterly. Last checked .
Comparing agencies
Pick three agencies and ask all of them the same things. Write the answers down. Here's what we'd tell you, so you have something to hold the others against.

In plain words
Search engines see everything Findlay families try. Home health care Findlay Ohio. Home health agencies in Findlay. A caregiver for mom. Private duty nursing. However you word it, the answer here is the same South Main Street office. Aides for personal care, nurses for the medical work, therapists when the doctor adds physical, occupational, or speech therapy, all of it scheduled through one Case Manager and all of it in your home, any day of the week. Call 419.422.0305 and just describe the situation. We'll take it from there.

Getting started
Tell us what's going on. We'll tell you whether home care, home therapy, or the outpatient clinic fits best, and we'll be honest if it's not us at all.
We're Medicare and Medicaid certified and work with Passport, waiver programs, and most private plans. Your doctor's orders set the plan.
Your aide, nurse, or therapist arrives on the ordered schedule, and a Case Manager keeps everything coordinated.
Findlay questions
For covered home health visits under Original Medicare, nothing out of pocket. Equipment is the exception at 20 percent after the Part B deductible, and Medicare Advantage plans write their own rules. Many Findlay patients come to us through Ohio Medicaid, Passport, or a waiver, and we take most private plans plus private duty arrangements. Bring your plan name to the first call and we'll walk it through with you.
Most starts happen within days. The clock runs on the doctor's orders and the coverage check, and nothing else. Call 419.422.0305 and we'll tell you honestly where your timeline stands.
Every week. Discharge planners send referrals through our referral form or by phone, and a Case Manager takes over from there. Families can start it too. If you're sitting in a hospital room right now, call us and we'll chase the paperwork.
Yes. Our aides handle bathing, dressing, meals, light housekeeping, and medication reminders, with a licensed nurse supervising the plan. Medicare only pays for aide visits alongside ordered skilled care, but Ohio Medicaid, Passport, and waiver programs cover ongoing daily help for many families, and private duty care is there when you'd rather pay for help directly. Call and we'll sort out which one fits your situation.
There are two tests and Medicare applies both. Leaving home has to be medically inadvisable or take help from another person or a device, and the patient has to be normally unable to leave home, with going out taking a lot of effort. It does not mean never leaving. Medicare allows trips for medical care, religious services, adult day care, and the occasional family event.
116 South Main Street in downtown Findlay. The phone side never closes. A Med1Care representative answers 24 hours a day, every day of the year, with no answering service in the middle. Most care happens in your home anyway, so come in only if you want to.
