What we do here

What we send, and who it's for

A doctor's order starts it. A Case Manager keeps it straight. You mostly just answer the door.

Personal care and support

Home care in Perrysburg without the guesswork

The aides we send are trained, supervised by licensed nurses, and picked for your household rather than pulled off a list. They cover the personal care, the meals, the light housekeeping, and the simple comfort of not being alone at home all day.

  • Bathing, dressing, grooming
  • Meal preparation
  • Light housekeeping
  • Help getting around safely
  • Medication reminders
  • Companionship

Skilled nursing at home

Skilled nursing across Wood County, delivered at home

For wound care, infusions, and the rest of the clinical list, our RNs and LPNs make the trip so your family doesn't have to. Physician orders drive every visit, and the reporting loop back to the doctor never drops.

  • Wound care
  • IV therapy
  • Injections
  • Diabetic management
  • Catheter care
  • Gastric feedings
  • Medication setup
  • Vital sign checks
  • Tracheostomy care
  • Respiratory care
  • Ventilator care

Physical, occupational, and speech therapy run through our in-home therapy team, and your doctor can add them to the same plan of care.

Paying for it

What Medicare actually covers

When you qualify, Original Medicare pays nothing out of your pocket for covered home health visits. Medical equipment is the exception, and that runs 20 percent of the approved amount after your Part B deductible. Medicare Advantage plans set their own cost sharing and often want prior authorization, so the answer there depends on your plan.

Qualifying takes a few things at once. A doctor has to certify the need after seeing the patient face to face, the care has to be part-time or intermittent skilled nursing or therapy, and the patient has to be homebound. We're a Medicare and Medicaid certified agency, and we also work with Ohio Medicaid, Passport, waiver programs, and most private insurers.

Don't fill anything out yet. Phone us at 419.866.0555 and we'll check your plan while you're on the line, then tell you what applies to your situation rather than the average one.

One straight answer up front.

There's no Medicare payment for help with bathing, dressing, meals, or housekeeping when that's the only care somebody needs. It covers aide visits alongside ordered skilled nursing or therapy, not on their own. Private duty care is available if you'd rather arrange and pay for help directly. If daily help is what your family is after, say so on the first call and we'll tell you what your options actually are, including the ones that aren't us.

Four questions that tell you where you stand

Nothing here gets sent anywhere. It's a gut check before you call.

Check the boxes that apply. Either way, the next step is the same phone call.

The record

What Medicare's own scorecard says about us

CMS scores every certified home health agency in the country on the same measures and publishes the results. Ours are below. Anyone can look them up.

5/5CMS Quality of Patient Care star rating
0%Falls with major injury
0%Pressure ulcers or injuries
100%Improved at bathing
100%Improved at breathing
100%Improved at managing their own medications
99.97%Care started on time

Two more worth knowing. Med1Care beats the national rate on discharge to community and on potentially preventable hospitalization, both risk-adjusted by CMS. Every figure here is published on Medicare Care Compare under CMS certification number 368362, where it refreshes quarterly. Last checked .

Comparing agencies

Nine questions to ask any home health agency in Perrysburg

Pick three agencies and ask all of them the same things. Write the answers down. Here's what we'd tell you, so you have something to hold the others against.

Is Med1Care Medicare certified?
Both Medicare and Medicaid. Our CMS certification number is 368362, and you can pull our full quality record on Medicare Care Compare.
Who actually comes to the house?
Aides handle personal care and daily support, working under the supervision of our licensed nurses. RNs and LPNs come for skilled care like wound care, injections, and IV therapy. Licensed therapists cover physical, occupational, and speech therapy through our in-home therapy team.
What are your hiring standards?
Licensed roles need the current credential, RN, LPN, STNA, or CMA, and we verify it during hiring. The Blade has named us a Top Workplace two years running, most recently in 2026, which matters here because agencies that keep their staff send you the same face twice.
Who sets the visit schedule?
Your doctor sets the frequency and a Case Manager builds the schedule around it. You'll know who is coming and when.
Can we reach a person after hours?
Always. The phone is answered 24/7/365 by a Med1Care representative, and we don't use an answering service. The office keeps weekday hours, 8:30 to 4:30. The phone never closes.
What payment sources do you take?
Medicare, Ohio Medicaid, Passport, waiver programs, and most private insurers. Private duty care is available too if you'd rather pay directly and skip insurance entirely.
Does our doctor stay in the loop?
The plan of care is built from your physician's orders, and the office reports back as the patient's condition changes it.
What if something isn't working?
Ring the office at 419.866.0555 and ask for the Case Manager on the account. If the match between caregiver and household isn't right, say so and we'll change it.
Who decides which caregiver we get?
The Case Manager does, working from your doctor's orders first and then from the household. Tell us what the week actually looks like, who else is home, and what went wrong with the last arrangement, and we'll match to that.

Perrysburg questions

What Perrysburg families ask us

How much does home care cost in Perrysburg?

If Original Medicare covers the visits, your share is zero. Equipment runs 20 percent past the Part B deductible, Medicare Advantage plans price their own way, and Ohio Medicaid, Passport, waivers, private insurance, and private duty fill the gaps for everyone else. We check your exact plan on the first call.

How quickly do visits start?

Once the doctor's orders are in hand, usually within days. Coverage verification is the only other gate, and we run it while you're still on the phone when we can.

Can you take referrals from Toledo-area hospitals for Perrysburg patients?

Yes, constantly. Perrysburg discharges come to us through the referral form and direct calls, and a Case Manager picks each one up and keeps it moving.

Can we get help with just bathing, meals, and housekeeping?

Yes. Our aides handle bathing, dressing, meals, light housekeeping, and medication reminders, with a licensed nurse supervising the plan. Medicare only pays for aide visits alongside ordered skilled care, but Ohio Medicaid, Passport, and waiver programs cover ongoing daily help for many families, and private duty care is there when you'd rather pay for help directly. Call and we'll sort out which one fits your situation.

What does homebound actually mean?

There are two tests and Medicare applies both. Leaving home has to be medically inadvisable or take help from another person or a device, and the patient has to be normally unable to leave home, with going out taking a lot of effort. It does not mean never leaving. Medicare allows trips for medical care, religious services, adult day care, and the occasional family event.

Where's your office for Perrysburg?

Our Holland office covers Perrysburg, and honestly you'll probably never see it. Visits happen at your home, and the phone reaches a live Med1Care representative 24/7/365, never an answering service.
